The Form 8 is the official “Application for Shifting of Residence / Correction of Entries in the Existing Electoral Roll / Replacement of Elector’s Photo Identity Card (EPIC) / Marking of Person with Disability (PwD)” according to the Election Commission of India (ECI). To make it even simpler: if you are a registered voter and you want to update your details or address or if your Voter ID card (EPIC) has been lost or damaged, then this is the form that you need to fill in.
When Would You Use Form 8?
Form 8 can be used by people where one of the following situations has occurred:
Shifting your residence: You have moved to a new house, a new constituency, and want your voter registration to be changed accordingly.
Correction of entries: If you discover that the name, date of birth, father’s name, or photo in the electoral roll is incorrect.
Replacement of EPIC: If your voter card (Elector’s Photo Identity Card) is lost, damaged, or misplaced, you can get a new one.
Marking of Person with Disability (PwD): If you are a voter with a disability and want your status to be indicated in the electoral roll.
How to File Application Using Form 8
First of all, you should download Form 8 from either the ECI website or your state CEO’s portal.
You then fill in your current details, which include your name, EPIC number, assembly or parliamentary constituency, etc.
You must point the correct position by ticking it under "I submit application for". The correct option (shifting, correction, replacement, PwD) should be marked.
By giving the information for the request you made, you can fulfill the need for that data — with a new address in the case of shifting, the correct details in the case of correction, the reason for replacement, and confirmation of disability for the PwD.
Apart from the application, you are expected to provide supporting documents such as address proof, a photo ID, and a disability certificate (if applicable). These documents must be attested by the person themselves.
You can hand over your filled-out Form 8 to the Electoral Registration Officer (ERO) or upload it on the online portal if your state approves this method.
You will get an acknowledgement number upon your submission; you can keep track of your application through it.
